Any thoughts on the following problem would be much appreciated.
After rendering my project on the timeline, using DC30plus and Premiere 5.1, the necessary Miro Instant temporary preview files are created. Howver sometimes when I close and reload the project the required preview files do not load. I can confirm however, that those files still exist in the temporary preview directory.

Don't know how to solve it, but it happens to me as well. The closest I've come to understanding it is my -- possibly wrong -- belief that any change to the project which moves a clip in the timeline by even a single frame makes Premiere throw out the old Instant video preview for that clip.
Instant video is a great idea, but unfortunately it also seems to be full of bugs.
I don't have this problem with the same setup. Are you using the 1.32 drivers for the DC30+. There is a 1.33 in beta but I haven't tried it and don't know the differences. Look on Pinnacle's European site - go to pinnaclesys.com and click the union jack
